🗓 Step 1: Book Your Appointment
You have selected the Blood Testing+ service. Selecting the Book Your Appointment button below, you will see a page similar to this one.
- For your treatment, after clicking the Book Your Appointment Now button, you’ll be guided through the scheduling process.
- Enter your contact information and select next.
- Select the Good Faith Exam Provider and choose a date and time available to complete your booking.
📧 Step 2: Watch for Three Important Emails
Once your appointment is booked, you’ll receive three key emails:
- Welcome Email
- Click the confirmation button to verify your account.
- A secure page will open where you'll confirm your profile details and address.
- You’ll need a mobile phone to receive and enter a verification code.
- Pre-Treatment Instructions (if applicable)
- If your selected service requires any prep, you’ll receive a detailed guide—please follow it carefully.
- Appointment Confirmation
- Confirms that your appointment is officially scheduled.
🔐 Step 3: Activate Your Account
After confirming your profile, you’ll receive another email prompting you to Activate Your Account:
- Click Activate to access your secure portal.
- Log in and complete your consent forms—these must be submitted before your visit.
📝 📍 Step 4: Complete Questionnaire
Since you purchased the Blood Testing+ service, after completing the profile in the electronic medical records (EMR) system you will see a questionnaire requesting you provide us the city and state where you wish to complete your blood draw.
Once you complete the questionnaire, our staff will email at least one blood draw center option in or near the city you provide.
📞📅💉📦🧾📷 Step 5: Schedule Blood Draw Appointment
- Call the blood draw center after the kit arrives.
- Schedule appointment.
- Confirm the collection fee.
- Bring the following to the appointment:
- Your collection kit
- The requisition form (provided by our team)
Reminder: Please verify how many tubes there are in your kit before attending your appointment and take a picture of all tubes in your kit for your records.
